Terms of Service

These terms are between you and Ocean Point Care LLC dba Origin Care Navigator (“Origin”). By using Origin Care Navigator you agree to them. If you do not agree, please do not use the service.

Origin provides care navigation information and decision support. Origin is not a healthcare provider, home care agency, licensed facility, attorney, or financial advisor. Nothing Origin produces is medical, nursing, legal, tax, or financial advice, a diagnosis, a treatment plan, or a clinical assessment. Always consult qualified professionals about medical, legal, and financial decisions.

Origin is not an emergency service. If someone is in danger, seriously injured, medically unstable, or at immediate risk, call 911 or go to the nearest emergency department. Do not use Origin, email, or text messages to report an emergency.

Care providers introduced through Origin are independent businesses. They are not employed, supervised, controlled, or credentialed by Origin, and Origin does not perform their services. We make reasonable efforts to work with licensed organizations in good standing, but we do not guarantee any provider’s availability, licensure, pricing, quality, conduct, or results. Any agreement for care is between you and the provider. Origin does not guarantee that a match, introduction, response, or placement will occur.

• Provide accurate information, and only information you have the right to share.
• Use the service for your own care decisions or for a person you are authorized to assist.
• Do not submit requests on behalf of someone who has not agreed to be contacted, and do not submit a phone number you are not authorized to use.
• Do not misuse the service: no scraping, resale, automated abuse, security probing, impersonation, spam, or unlawful, harassing, or deceptive activity.
• Providers must maintain required licensure and respond to referral offers honestly.

Text messaging is optional and is never a condition of receiving services or of participating in Origin’s provider network. Consent is collected through its own separate, unchecked checkbox. Origin sends recurring transactional messages only.

• Reply STOP to any Origin Care Navigator text message to opt out of further messages. Reply HELP for help. Message frequency varies. Message and data rates may apply.
• To stop messages, reply STOP. Origin records the opt-out and suppresses further texts to that number. For help, reply HELP or email support@origincarenavigator.com.
• Carriers are not liable for delayed or undelivered messages.
• SMS consent and mobile information are never sold or shared with third parties for their own marketing or promotional purposes.

The service is provided “as is” and “as available,” without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, accuracy, and non-infringement. To the maximum extent permitted by law, Origin is not liable for indirect, incidental, special, consequential, or punitive damages, or for the acts, omissions, services, or fees of any independent provider. Our total liability arising from the service will not exceed one hundred US dollars ($100). Some jurisdictions do not allow certain limitations, so parts of this section may not apply to you.

We may update these terms; the effective date above will change. We may suspend or end access for misuse. These terms are governed by the laws of the State of California, without regard to conflict-of-laws rules, and disputes will be heard in the state or federal courts serving Orange County, California.
